CHANGE OF NAME IN THE PASSPORT

The following cases are to be considered as name change:-

(i) Married woman wishing to revert to maiden name when not divorced or Widowed

(ii) Change of name upon attaining adulthood from childhood

(ii) Minor change in name and

(iv) Change/addition of surname

Check list for the required documents

• Print out of duly filled online Indian Passport Application Form

at https://embassy.passportindia.gov.in/.

• Two recent good quality passport size (2 inch x 2 inch) coloured photographs with frontal view of the full face with white/light background with dark coloured dress. (One photo must be pasted at specific place with the application and one extra photo attached also)

• Current passport in original and their self - attested copies (first & last page)

• Self - attested copy of Residence Permit/Work permit (front & back side)

• (Applicant with temporary residents and visitors/tourists from India or third country - original newspaper announcement announcing change of name, published in two leading daily newspapers. One newspaper should be of area of applicant's permanent address in India and the other of the area of his present address in Sweden/Latvia. Publishing should be in English. E-paper announcement aren’t acceptable. (Applicant and their dependent family members who are on long term permanent or work visa/permit may submit two public documents issued by the local government/local school/educational institution where the student is enrolled along with proof of work visa/Permit)

• Latest Person Bevis issued by Skatteverket for residents of Sweden in English

• One local address proof for residents of Latvia in English

• Proof of payment of (SEK 792 for 36-page booklet) or SEK 1052 for 60-page booklet) for above 18 years (SEK 534 for minor up to the age of 15 years)

• A self - addressed stamped REK envelope- if you want the passport to be posted back to you (Applicants living in Latvia may send self-addressed envelope with 11 International Reply Coupons)

CHANGE OF NAME FOLLOWING A MARRIAGE

Upon marriage, if the Indian women citizens wish to change their names (change of given name or change/addition of surname), they can do so by following a procedure of submitting the following documents with the Embassy:

• Two recent good quality passport size (2 inch x 2 inch) coloured photographs with frontal view of the full face with white/light background with dark coloured dress. (One photo must be pasted at specific place with the application and one extra photo attached also)

• Current passport in original and their self - attested copies (first & last page)

• Self - attested copy of Residence Permit/Work permit (front & back side)

• Self – attested Marriage Certificate issued by Registrar of Marriage (attested by the Home Department of concerned State Government and then by the Consular Division in the Ministry of External Affairs, New Delhi). If issued in Sweden/Latvia, the marriage certificate should be duly apostilled. Marriage certificate in Latvian should be translated in English.

• Latest Person Bevis issued by Skatteverket for residents of Sweden in English

• One local address proof for residents of Latvia in English
