2.Two passport-size (2 inch x 2 inch) recent colour photographs with frontal view of the full face with white/light background with dark coloured dress and one photo to be pasted on the application form

3. Current passport in original along with self-attested copies of the same (first & last page)

4. Self-attested copy of lost/damaged passport (first & last page)

5. Self-attested copy of Residence Permit/ Work permit front & back side

6. Self-attested copy of Proof of Address (if there is a change)

7. Proof of payment of SEK 172/- Bank Giro No. 5816-4930

8. A statement by the applicant explaining circumstances in which passport was lost/damagedâ 

9. Copy of confirmed ticket to India
